Gambling disorder
Overview
Persistent, recurrent problematic gambling causing significant impairment — a behavioural addiction sharing features with substance dependence (preoccupation, tolerance, chasing losses, loss of control, harm).
Recognise
- Preoccupation; needing to bet more (tolerance); chasing losses
- Lying about extent; jeopardising relationships/job
- Restlessness/irritability when cutting down (withdrawal-like)
Red flags
- High comorbid depression and suicide risk; severe debt/social crisis
Differentials & how to tell them apart
Manic episodegambling occurs only during mania with other manic features
Substance use disordersubstance-driven; may coexist
Investigations
Screening (e.g. PGSI). Assess comorbid mood, substance use and suicide risk.
Management
- 1CBT and structured support (e.g. NHS gambling clinics, GamCare). Treat comorbid mental illness.
- 2Debt/social support; address suicide risk.
No licensed drug — treat comorbid depression/anxiety; manage suicide risk
Key points
Strong association with depression and suicide — always screen.
Monitor & prognosis
Gambling behaviour, mood, suicide risk, financial/social harm.
Relapsing; CBT helps.
