Why this matters on day one

Foundation training is the point at which you stop being a learner attached to a team and become a functioning part of one. This outcome covers handover, referral, MDT working, safety, QI and using health systems.

What the domain requires

1. 1Work in the team. Handover, referral, MDT communication, and understanding what every other profession on the ward actually does.
2. 2Use the system. Navigate primary, secondary, community and social care, and understand how resource constraints shape what is deliverable.
3. 3Improve safety. Report incidents, contribute to reviews, complete an audit or QI cycle, and understand human factors.
4. 4Use information well. Records, IT systems, information governance and data protection.

What evidencing it looks like

• A completed audit or QI cycle.
• Multi-source feedback from the whole MDT.
• Incident reports and exception reports.

What falls short

• Never having spoken properly to the ward pharmacist, physiotherapist or social worker.
• A QI project with no measurement.
• Not exception-reporting, and then complaining the rota is unsafe.
