Why this matters on day one

Split into three parts that trainees weight very unevenly: clinical knowledge (over-revised), the health service and its structures (under-revised), and legal and regulatory obligations (usually learned only after a problem).

What the domain requires

1. 1Clinical and biomedical knowledge. Applied to the patient in front of you, including how disease presents differently in the frail, the very young and the immunosuppressed.
2. 2Health service and systems. How UK care is commissioned and delivered, pathways between primary, secondary and social care, and where the bottlenecks that affect your discharge actually are.
3. 3Legal and regulatory. Capacity, consent, confidentiality, safeguarding, death certification, the Mental Health Act, and your obligations to the GMC and your employer.

What evidencing it looks like

• Exam performance broken down by domain.
• Evidence of understanding pathways — a discharge or referral you organised end to end.
• Reflection on a case with a significant legal or ethical dimension.

What falls short

• Knowing the disease and not the pathway, so a correct plan cannot be delivered.
• Learning the law reactively, after a capacity or confidentiality problem has already been handled badly.
