Why this matters on day one

This is what the CPSA samples directly, station by station. Every item here is a performance rather than a fact, which is why it responds to rehearsal and not to reading — and why Finalist has a spoken station simulator at all.

What the domain requires

1. 1Communication and consultation. Take a history that captures the person as well as the disease, share decisions, break bad news, handle distress and conflict, and work with interpreters and advocates.
2. 2Diagnosis and management. Form a prioritised differential, choose and interpret investigations proportionately, and construct a management plan for acute, chronic and preventive care.
3. 3Practical procedures. Perform the specified procedures to day-one standard, with consent, asepsis and documentation — and decline what is beyond you.
4. 4Prescribing. Prescribe safely and legally, monitor effects, detect adverse reactions, and adjust for renal function, pregnancy, age and frailty.
5. 5Emergency care. Recognise and manage the acutely unwell patient, provide immediate life support, and escalate in time.

What evidencing it looks like

• Directly observed procedural skills (DOPS) sign-offs against the 23 GMC procedures.
• Mini-CEX or CBD entries on acute presentations, including one where the diagnosis was not obvious.
• A prescribing review you performed that changed a chart.
• Simulation and station practice logs — including the cues you habitually miss.

What falls short

• Practising history-taking by reading frameworks rather than saying them out loud under time.
• Collecting procedure sign-offs at the minimum count and never repeating them.
• Being able to describe management but not to prioritise it when three patients need something.
• Treating prescribing as an afterthought when it is the commonest source of FY1 harm.
